What Is Advance Care Planning?

Advance Care Planning (ACP) is a gift to you and those you love. ACP provides clear guidance of an individual’s goals, wishes and values as it pertains to current and future healthcare. Advance Care Planning starts with a conversation with you and your loved ones or healthcare agent. By creating a plan, you identify and document the kind of care you want when when you no longer have the ability to communicate your wishes. To begin this process, please complete the information under “Getting Started with Advance Care Planning” and a member of Radiant’s Advance Care Planning team will contact you.

Transitions Program

Education and Support for Those with Advanced Illness. Radiant’s Transitions program offers emotionally supportive phone calls, volunteer companions and referrals to community resources for individuals and families who are dealing with advanced illness and need additional support. Supportive phone calls to individuals and caregivers. Volunteer assistance for those who can benefit from companion visits. Information about and referrals to community resources. Education to help with understanding care options and choices. Advance care planning inclusive of advance directives. These services are offered at no charge.

Hospice Care

We are offering compassionate care and support to individuals with life-limiting illness and their families, which address the physical, psychosocial, emotional and spiritual needs associated with advanced illness while respecting each individual’s goals, beliefs and wishes. Radiant’s team of medical providers and staff will expertly manage pain and symptoms at home or in a variety of settings including hospitals, nursing homes or assisted living facilities.Hospice Care May Be Appropriate if an Individual is… Diagnosed with an advanced Illness. Losing weight but not dieting. Calling the doctor more frequently than usual. Confused and can’t get his or her medications right. Making frequent visits to the emergency room over the past six months. Falling frequently. Experiencing pain, even with prescribed pain medications. Using oxygen most of the time. Needing help with three or more of the following: Getting in and out of bed. Getting dressed. Eating. Bathing. Walking.

Grief Support

Healing Hearts Bereavement Support. After the loss of a loved one, coping with daily living can be hard. Knowing you are not alone in your difficult journey can make all the difference. Through generous community donations, Radiant offers grief support in 58 counties across California as a community service. Radiant offers individual, family and group bereavement support, as well as educational programs for anyone who has experienced a loss. One-on-one counseling is currently conducted via telehealth on the phone.
